Forms/Worksheets

• College Worksheet - Use this form to keep track of important information about colleges you want to apply to.
• Student Brag Sheet - All seniors must turn this in to their counselors before their Senior Interview in the fall.
• Parent Brag Sheet - All parents must turn this in to their student's counselor in the fall of the senior year.
• Alumni Transcript Request Form - Alumni may use this form to request an official transcript be sent to an employer or college/university.   Please note that official transcripts can not be mailed/given directly to the student.
• Release of Records Form - This form allows the Guidance Office to send out the student's academic information to college admissions departments and athletic offices.    Without this form, the parent and student must sign every orange cover sheet for college applications and must give written permission for every athletic recruiter that requests information.   Both the parent and the student must sign this form.

*The application orange cover sheet and blue teacher recommendation form are not available online.  Students must pick them in the Guidance Office and submit the original, colored forms.  Photocopies will not be accepted.

• Junior Night Booklet (pdf) - "A Guide to the College Selection and Application Process 2007-2008".

  This booklet was created by the BG Guidance Department as a two year guide for students and parents.  All Juniors received a copy from their counselors in school.  All parents who attended Junior night were also given a copy.
